Dietary Restrictions

Dinner will be a buffet this year. If your son or his date has any dietary restrictions that the Marriott needs to be made aware of please include them in the appropriate fields when competing the online registration. CBA will notify the Marriott and they will make the appropriate changes.

Dance Guest Authorization Form

Dance Guest Authorization Forms need to be completed by the date’s parent and/or guardian and submitted through the Google Form by Monday, January 12th.

Illness Protocol

If your son or his date becomes ill and is unable to attend your son can still attend as a single for the price of $100.00 or a refund will be issued. Please notify Dr. Lewyckyj as soon as possible if this is the case so we can make the appropriate arrangements.
